Complete the document located at Raffle Application (doc) prior to any raffle being held. Return the completed application to the CPC Business Office, Attention Campus Programs. You will not be able to hold your raffle until you receive notification that your application has been approved.

Within two weeks of completion of the raffle you must go to the link called State Document-Final Raffle Report (pdf) and complete the form. Return this form to the CPC Business Office, Attention Campus Programs, and it will be submitted by our office to the appropraite state agency.
